The Texas Fire Chiefs Association (TFCA) and the State Fire Marshal's Office (SFMO) have been working with the Insurance Services Office (ISO) to review and update the questionnaire used to determine credit under the ISO Texas Addendum.

The approved questionnaire is on-line at http://www.tdi.state.tx.us/fire/fmppcfaq.html. It is now available in a form that allows computer completion.

Chiefs Lonzo Wallace, Richard Lazarus, Cleve Calagna, and Harry Simms met with ISO and SFMO representatives to iron out the details for this questionnaire.
